In normal delivery, 2.7 million mothers worldwide experience a tear in the birth canal. The causes of birth canal lacerations that occur during labor are caused by several factors, including delivery position, how to push, delivery leadership and newborn weight. The purpose of midwifery care is to implement a supervisory position through comprehensive midwifery care management and SOAP documentation. This care uses a descriptive observational design with a case study approach which was carried out at the “W” Midwife Independent Practice in Padang City on April 15, 2021. The subject of this care is Mrs. Z G4P3A0H3 39-40 weeks of gestation. Data were collected using primary and secondary data. The tools used are the maternal format, weight scale, blood pressure meter, stethoscope, clock, thermometer, height measuring tape, lila tape, meter (fundal height measurement), PPE, doppler, labor set, partograph, heacting set, and handscoon. The results of midwifery care for Mrs. Z with the application of the position of the pusher in the "W" Midwife Independent Practice was successful because there was no perineal rupture in the mother. During the provision of care, no complications were found and all normal delivery care could be optimally provided to the mother. The conclusion of the application of care in the semi-sitting position for pregnant women is effective in preventing rupture of the perineum during labor. It is hoped that independent practice midwives can apply care in a half-sitting position for maternity mothers.
